Andrzej 'Andrew' Bernatowicz (1873 - 1918) - Find A Grave Memorial

After a few edits and requests submitted to the Find A Grave contributor for revisions for this memorial, it has finally been corrected.  The given name was incorrect.  

The surname on the gravestone is spelled "Bernatawicz" however the death certificate and the cemetery record show "Bernatowicz."  

It has been verified also that "Bernatowicz" is a valid Polish surname:
